Vacancy 1: Carers Connections Worker

Hours: Full time, 35 hours per week
Salary: SCP12 £21,589
Location: Leeds city centre
Closing date: 4 pm Thursday 20 August 2020
Interview date: Friday 04 September 2020

Carers Connections is a project supporting older carers (aged 50+) who are lonely or socially isolated. The project is funded by Time to Shine (Big Lotteries: Ageing Better Programme).

The post holder will offer support to carers with the intended outcome of increasing sustainable social contact, community participation and new relationships. Mindful of the needs of the diverse community of carers in Leeds, support will be delivered across the city either in person or via digital platforms such as Zoom.

The post holder will also contribute to the overall aims of the Time To Shine project timetoshineleeds.org through supporting research and evaluation.

Vacancy 2: Hospital Liaison Support Worker

Hours: 21 hours per week
Salary: SCP23 £26,999 (£16,199 actual salary)
Contract: Temporary – 1 year fixed term
Closing date: 4pm Friday 21 August 2020
Interview date: w/c 31 August 2020

The post of Hospital Liaison Support Worker is funded for one year to work with carers and people with care needs who are setting up self-funded care provided by the independent sector. Working mostly within Leeds Teaching Hospitals Trust (LTHT), carers and the person with care needs, will be supported in making a smooth transition from hospital, into residential care or care in their own home.

The post holder will be someone with knowledge and understanding of carers issues and an ability to make good working relationships with LTHT and independent care providers.

This post has been created in partnership with the Leeds Care Association, the independent sector representative body for care providers in Leeds.
